The accident began when a black Ford Mustang, driven by Audrey Champlin (21 years of age of Fayetteville, AR) was traveling east on Hwy 412 approaching the intersection of Mt. Olive and failed to avoid a collision with a silver Honda Accord, driven by Lawrence William (38 years of age of Siloam Springs) which was stopping for traffic to its front. As a result of the collision the Honda then struck a Peterbilt tractor-trailer, driven by Ronald Davis (52 years of age of Bolivar, MO) which was traveling east, causing the driver to lose control of the vehicle. The tractor-trailer traveled northeast through the intersection dragging the Honda, resulting in the Honda striking a white Chrysler PT Cruiser, driven by Raymond Baltas (20 years of age of Baton Rouge, LA). The tractor-trailer then struck a Ford Freestar van driven by Robert Jones (38 year of age of Siloam Springs, AR), dragging the Freestar van northeast through the intersection where both vehicles struck the Centennial Bank drive-thru eventually coming to rest.

A passenger in the Ford Freestar was transported to Washington Regional Hospital for minor injuries and the occupants of the Honda were transported to Siloam Springs Regional Hospital with minor injuries.

U.S. Hwy 412 was closed for several hours as Siloam Springs Police investigated the accident and Benton County Sheriff's Deputies assisted with traffic control while traffic was rerouted around the accident scene.

The driver of the Mustang was issued a citation for following too close.
